Your initial appointment will consist of meeting Doctor Smith and his experienced team to answer all of your questions about Orthodontic treatment. Beautiful smiles begin with a complete examination of the teeth, jaws, bite, and soft tissue facial balance. Dr. Smith will provide you with all treatment options as well as the appropriate timing concerning the start of treatment. Also, he will give you details about the expected length of treatment, cost, comfort and of course, options for how best to customize and treat your case. If the time to begin orthodontic treatment is right, he would suggest taking a set of orthodontic diagnostic records which include: molds of the teeth, photographs, and special orthodontic X-rays. These orthodontic records with allow Dr. Smith to study in depth the best solution to create a beautiful smile for you or your child. Every patient is different and thus every patient will get a customized treatment plan to best suit their needs. No "one size fits all assembly line here!" Dr. Smith will give you alternatives and options for different types of braces or clear aligners to produce the best comfort, speed, and efficiency for the result you are after.
If you have dental insurance, bring your current insurance card. If you don't have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.
IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.).
X-Rays
If your dentist or physician has taken X-rays, please have them forwarded to our office as they may be helpful in our diagnosis. A digital radiograph can be emailed to us. If additional films are necessary, they can be taken at our facility.
